Medication Clarity™ provides private-pay clinical pharmacy consultations for patients and caregivers. Consultations are designed to help clients better understand their medications, organize medication information, and prepare informed questions for their healthcare providers.

2. Eligibility

​

You must be at least 18 years of age to book a consultation.

​

A parent, legal guardian, or authorized caregiver may schedule a consultation on behalf of another individual when appropriate.

3. Scope of Services

​

Services may include:

• The Transition Home Medication Review™

• Comprehensive Medication Review

• Caregiver Medication Consultation

 

Consultations are educational and informational in nature and are intended to support informed discussions with your healthcare providers.

4. Professional Disclaimer

​

Medication Clarity™ consultations are not a substitute for medical care.

​

Services provided through Medication Clarity™:

• Do not diagnose medical conditions.

• Do not prescribe medications.

• Do not replace care provided by your physician or other licensed healthcare professionals.

• Do not establish a physician-patient relationship.

 

Clients should never start, stop, or change medications without consulting their prescribing healthcare provider.

 

If you are experiencing a medical emergency, call 911 or seek emergency medical care immediately.

 

 

5. Payment

 

Medication Clarity™ consultations are private-pay services.

 

Payment is required at the time of booking.

 

Insurance, Medicare, and Medicaid are not accepted.

 

 

6. Cancellations & Refunds

 

Your appointment is reserved exclusively for you.

 

Please refer to our Refunds, Cancellations & Appointment Policy for complete details regarding cancellations, rescheduling, refunds, and no-show appointments.

 

 

7. Client Responsibilities

By scheduling a consultation, you agree to:

• Provide accurate and complete information.

• Complete the Medication Clarity™ Preparation Form before your appointment.

• Upload requested documents by the stated deadline.

• Attend your consultation at the scheduled time.

 

Medication Clarity™ is not responsible for recommendations based on incomplete or inaccurate information provided by the client.

 

 

8. Intellectual Property

 

All website content, consultation materials, Medication Clarity™ Reports, logos, graphics, text, educational resources, and other materials are the intellectual property of Tamar Lawful, PharmD unless otherwise noted.

 

You may not reproduce, distribute, modify, publish, or commercially use any content without prior written permission.

To the fullest extent permitted by law, Tamar Lawful, PharmD shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of this website or consultation services.
