The Transition Home Medication Review™

Designed for patients recently discharged from the hospital who want to better understand medication changes before their follow-up appointments.

Comprehensive Medication Review

A complete review of prescription medications, over-the-counter medications, and supplements for individuals taking multiple medications or seeking a better understanding of their medication regimen.

Caregiver Medication Consultation

Designed for family members helping an aging parent or loved one manage multiple medications.

How do I know which consultation is right for me?

Choose The Transition Home Medication Review™ if you:

  • Recently left the hospital

  • Had medications started, stopped, or changed

  • Feel overwhelmed by discharge instructions

  • Want to prepare for your follow-up appointment

Choose the Comprehensive Medication Review if you:

  • Take several medications

  • Have questions about your medication regimen

  • Want to better understand why you're taking each medication

  • Want to review prescription medications, over-the-counter medications, and supplements together

Choose the Caregiver Medication Consultation if you:

  • Help manage medications for a parent or loved one

  • Feel overwhelmed by multiple medications

What can I expect during my consultation?

Every consultation includes:

  • Review of your completed intake form

  • Review of your medication list

  • One-on-one virtual consultation

  • Time to discuss your questions and concerns

  • Personalized recommendations for topics to discuss with your healthcare provider

  • A written medication summary

  • An updated medication list

  • Secure follow-up within seven days

 

The focus of the discussion will depend on the consultation you've selected.

 

How long are consultations?

Each consultation lasts approximately 60 minutes.

How much does each consultation cost?

All consultations are currently offered at the same flat rate.

$249

This includes your pre-appointment review, virtual consultation, written medication summary, updated medication list, and secure follow-up.

What is included in my written medication summary?

Your personalized summary may include:

  • Your current medication list

  • Medication changes discussed during your consultation

  • Tips for staying organized

  • Important questions to discuss with your healthcare provider

  • Space for your own notes during future appointments

 

 

Is this a replacement for my doctor?

No.

These consultations are designed to help you better understand your medications and prepare for conversations with your healthcare providers.

I do not diagnose medical conditions, prescribe medications, or replace your physician or primary pharmacist.

Will you change my medications?

No. Any medication changes should always be made in consultation with your prescribing healthcare provider. If questions or concerns arise during your consultation, we'll discuss them together so you can have an informed conversation with your physician.

Can you identify medication interactions or duplicate therapies?

As part of reviewing your medication list, we may discuss potential medication-related concerns that could be important to address with your healthcare provider.

The purpose of the consultation is to help you better understand your medications and prepare informed questions—not to replace medical care or independently change your treatment plan.

 

Are appointments virtual?

Yes.

All consultations are conducted through a secure virtual video appointment, allowing you to participate from home.

What should I have ready before my appointment?

Depending on your consultation, please gather:

  • Your medication list or medication bottles

  • Vitamins and supplements

  • Hospital discharge paperwork (for Transition Home consultations)

  • Recent medication changes

  • A list of questions you'd like answered

Do you work directly with my physician?

No. You're welcome to share your medication summary with your physician, specialist, or other members of your healthcare team if you believe it would be helpful.

Who can benefit from these consultations?

My services are designed for:

  • Adults managing multiple medications

  • Patients transitioning home after hospitalization

  • Individuals living with chronic health conditions

  • Family caregivers

  • Anyone seeking a better understanding of their medications

 

Where are consultations available?

Consultations are currently available for clients located in California. As my practice grows, additional states may become available.

Is my health information private?

Yes. Protecting your privacy is a priority. Your personal health information is handled securely and in accordance with applicable privacy laws and professional standards.

What if I need to cancel or reschedule?

Appointments may be rescheduled with at least 24 hours' notice. Please see the Scheduling & Cancellation Policy for complete details.
