Leaving the hospital can feel overwhelming. New medications, discontinued medications, dosage changes, and follow-up appointments can make it difficult to know exactly what you're supposed to be taking. This one-on-one consultation is designed to help you better understand your updated medication regimen after a hospital stay. Together, we'll review your discharge medications, discuss any changes that were made, answer your medication questions, and prepare you for your follow-up appointments. This consultation may be right for you if you: 1. Were recently discharged from the hospital 2. Had medications started, stopped, or changed 3. Have questions about your discharge medication list 4. Want to feel more confident before your follow-up appointment Your consultation includes: 1. Pre-appointment review of your medication information 2. 60-minute virtual consultation 3. Review of prescription medications, over-the-counter medications, and supplements 4. Personalized Medication Clarity™ Report 5. Updated medication list 6. Questions to discuss with your healthcare provider 7. Secure follow-up within seven days Please have your hospital discharge paperwork and current medication list available before your appointment. PAYMENT & INSURANCE Medication Clarity™ consultations are private-pay services. Payment is due at the time of booking to reserve your appointment. Insurance, Medicare, and Medicaid are not accepted, and these consultations are not billed to insurance. If requested, you will receive a receipt for your records. Please note that reimbursement from your insurance company is not guaranteed.
